By signing this form, I hereby grant to MFPS and its Affiliate Programs or Mentored Regions permission to reproduce and distribute the whole or any part of my work or my team’s work for educational or promotional purposes, and allow MFPS to license or sell such excerpts, in any form or medium, without any further permission and without any financial obligation to me. I also grant permission to allow any photographs or audio or video presentations or recordings taken or made during my on-site or virtual attendance at MFPS-related events, including the State Bowl and International Conference, and in which I appear, to be used for educational or promotional purposes under the terms outlined above. This consent to a royalty-free license is a condition of participation in all FPSP-sponsored events. I understand that if I do not agree to these license terms, MFPS and its Affiliate Programs and Mentored Regions cannot accept my application to participate in State Bowl events. This is a nonexclusive license. The authors of written work retain all rights to their own work (except the right to terminate this license), including the right to publish or submit to others for publication. Statement of Authenticity: In addition, by signing this form, I certify that my writing or my team’s writing as the case may be, is original work and the sole creation of a student’s(s’) own efforts and ideas. Plagiarism is an offense defined as the use of another’s words, ideas, or expressions without acknowledging their source. I further understand that plagiarism would be grounds for immediate disqualification from MFPS competition. |
This contract will apply if you advance to State Bowl/Competition or the International Conference (in each case either on-site or as a virtual event). Violation of any rules may result in individual or team disqualification and/or competitor(s) being sent home at personal expense or denied further virtual access.
FPS offers unique opportunities and experiences, both educationally and socially. By using GOOD COMMON SENSE and RESPECT AND CONSIDERATION FOR OTHERS AND THEIR PROPERTY, you will be able to have an educational, safe, and enjoyable experience at our events. By signing this agreement, you agree to comply with the following rules of behavior.
∙ Comply with the rules of the various agencies involved with the event: MFPS, transportation providers, onsite facilities, etc. ∙ Periodically review and abide by any terms of use and privacy policy posted on MFPS website in which you participate
∙ Take financial responsibility for any damages incurred
∙ Not use abusive, harassing, profane or other inappropriate language
Parent Agreement( If competitior is under 18)
I have read and support the competitor rules and regulations of MFPS. My child or ward has read the rules and regulations and will comply with all of these rules and regulations. It is understood that the signature on this behavior contract of one parent or guardian implies the consent of the other
